Payment Information

We understand that sometimes the payment part of a patient visit can be more complicated than receiving treatment. Our friendly staff is experienced and more importantly, knowledgeable and current on the many issues surrounding insurance and payment options.

Here are a few items which may be helpful to you...

• Please bring your insurance cards including any required insurance referrals to each visit.

• Please be prepared to pay your required co payment at the time of your visit as per your individual insurance coverage policy. (We are required to collect these.)

• We participate with the following insurance carriers, however it is essential and your responsibility that YOU VERIFY YOUR COVERAGE with your carrier as the industry and individual plans have frequent changes.

• Patients with no insurance coverage or coverage with a non-participating insurance carrier should be prepared to make payment at the time of each visit. Our office billing staff is available to you at each visit to discuss your payment options.

• We accept cash, checks and major credit cards as forms of payment.
